Our Clinical Consultant Pharmacists play an integral role in the lives of residents in Assisted Living Facilities through the optimization of medication use. Whether its through identification of unnecessary medications, the identification and prevention of drug interactions, or recognizing adverse drug events, appropriate interventions by our Pharmacists extend quality of life by helping these patients age in place.
Each Facility receives on-site visits from the Pharmacist on a monthly or quarterly basis, as determined by the needs of the residents. All issues identified are reviewed with the appropriate staff and specific recommendations for medication therapy changes are forwarded to each individual's private physician for evaluation and implementation, where appropriate.
