Adult Day Health Care provides unique challenges and opportunities for our Pharmacists. Through thorough Drug Regimen reviews, conducted on a monthly, quarterly, or semi-annual basis for every person who attends the program, we are able to identify drug therapy concerns, and make recommendations to improve outcomes.
From experience, we know that many of the people that attend Adult Day Health Care programs use multiple physicians and multiple pharmacies. This places us in the unique position of being the only professionals with an opportunity to see and evaluate each patient's total medication profile, and allows us to help prevent an array of potential medication problems.
